You may have seen a form being passed around or shared or retweeted that looks like an official Washington State Department of Health exemption form for personal and religious reasons. This form has several checkboxes - one being one for Covid-19. Don't fall of it as it's fake. It was edited from another form but someone added the Covid-19 checkbox hoping to get away with it. The Department of Health said you don't need a paper form for this.

When looking for clarification on this form, and to give them a head's up since it's being passed around, here's what they said.

"An adult who does not want to get the COVID-19 vaccine does not need to get an exemption form from the Department of Health. Washington state only uses certificates of exemption for vaccines required for children in K-12 schools and child care so facilities know who is at risk during a disease outbreak. Currently, Washington does not require the COVID-19 vaccine to attend school or child care.

"If your employer or college/university requires the COVID-19 vaccine, or you are required to have the vaccine per Gov. Jay Inslee's Aug. 9 proclamation, you should contact your employer or college/university to find out how they collect proof of vaccination, if they have an opt-out policy, and what you would need to do to opt out. It is up to employers and schools to determine their policy."

If you must opt-out for any reason, contact the one mandating directly to see what options you may have. When in doubt, contact the Washington State Department of Health directly.
